If you have opened the link in your web browser (simply click the link), first fill out the form, then click "Save a copy" (circled in red below), and then rename the file and choose the location (usually the desktop is a good location). Then send us the new renamed file.

Rename the file as shown, (usually the desktop is a good location). Then send us the new renamed file.
